New Patients
How to Register with our Practice
Please collect the necessary paperwork from the Practice, which
consists of
a registration form and a questionnaire. (Or you can download the
forms via the links below.) Fill the forms in and pass them back to
reception. It is often several months before we receive your medical
records. If you wish you can make an appointment with a Sister
for a routine health check, which includes taking your height,
weight and blood pressure. You are only formally accepted when the
paperwork has been signed. You will receive a letter confirming your
registration.
If you are on regular medication, please make an appointment to see
one of the GP's for a medication review.
