Vision for Health Care at Grosvenor and Gresty Brook Medical Centres.
We intend to provide local medical and social care in the communities where people live. We will provide open access to GPs and other professionals, a personal doctor to explain and help joint decision making and use the practice list system to offer advice before illness develops and management of chronic disease if it does. We aim to provide care for common illnesses most of the time at the surgery without referral. We aim to offer a higher level service in mental health, elderly care and rehabilitation and link to our district nurses to provide care in the home. We aim to help integrate the care of different illnesses and aid the understanding of patients about services delivered elsewhere by specialist departments.
We wish to work with other GP surgeries to provide a greater range of services and to locate voluntary and other community services around our surgeries.
